First Visit Guide
Thank you for entrusting me with your spinal care. In order to make your first visit as pleasant and friction-free as possible, we have outlined some important information below.
- Have a referral faxed over to our office (if needed) from your primary care provider.
- Ask your previous place of care or provider to fax over the two most recent office visit notes.
- Bring your most recent imaging (MRI, CT, Xray) on a CD disc. This is not required if imaging was performed at a Shields facility or at New England Baptist Hospital.
- Print (if applicable) and bring the completed forms we have sent you in the mail or email. Please be sure to be thorough with the forms, as they are imperative to your care.
- Inform us if you have an open worker’s compensation claim or have sustained your injuries in a motor vehicle accident, as we have special preparations for these visits.
If you need to reschedule your appointment, please give us at least 2 days’ notice to allow us time to move other urgent patients up into your time slot. If you are running late, please call ahead of time.
*The office reserves the right to cancel your appointment if you are 30 minutes late to your scheduled time to ensure maximum care for all our scheduled patients.
- We ask all new patients to arrive at least 30 minutes early for your appointment for necessary radiographs.
- Upon arrival, please check in with Patient Access in the lobby and inform them of your scheduled radiographs (X-rays) and appointment with Dr. LeRoy.
- Please remember to bring a Photo ID and Insurance cards with you to your appointment.
